Liability Waiver Agreement

I voluntarily choose to receive counseling or coaching services from Warriors in Christ, operated by a Georgia Certified Christian Counselor. I understand that these services are not a substitute for medical, psychiatric, or psychological treatment and do not involve the diagnosis or treatment of mental disorders as defined by the American Psychiatric Association.

I have been thoroughly informed about the nature of counseling and coaching services, the methodologies employed, and the limits of confidentiality. I acknowledge that while the services aim to facilitate personal growth and development, the outcomes cannot be guaranteed.

I am aware that discussing personal and sensitive issues during counseling or coaching sessions may cause emotional or psychological discomfort. I assume all risks associated with these services and understand that all decisions made during these sessions are entirely my own.

I hereby release Warriors In Christ, its counselors, coaches, and staff from all liability for any personal injury, psychological or emotional damage, or any other changes that may occur as a result of my participation in these services.

I have been advised of my right to consult an attorney before signing this agreement to ensure that I fully understand its terms and conditions.

I understand that I have the right to revoke this consent in writing at any time, except to the extent that action has been taken in reliance on it.

This agreement shall be governed by and construed in accordance with the laws of the State of Georgia. Any legal disputes arising from this agreement or the services provided will be resolved through mediation or arbitration prior to any court action.

By checking this box, I affirm that I am over the age of 18, have read and understood the terms of this liability waiver, and consent to its conditions voluntarily and without any duress or undue influence. I acknowledge my commitment to the counseling or coaching process and my responsibility for my personal growth and development.
